Getting started with long-term care information
Where to begin locally and how to keep property decisions flexible while care questions are explored.
Begin with Fraser Health
New clients can contact Fraser Health’s Home and Community Care Access Line or use the telephone appointment form on its website. Current clients should contact their Home Health team. A community health professional can explain the assessment process and local publicly subsidized access.
Questions for the care team
- Who will explain the assessment and the next steps?
- How will my preferred location and daily routines be considered?
- What happens if my needs change while I am waiting?
- What support may be available at home?
- Who explains costs, included services and the information required?
Confirm the funding pathway
Ask whether the enquiry concerns publicly subsidized or private-pay care. For a private-pay enquiry, confirm services, access and suitability directly with the provider and appropriate health professionals. A residence’s operator name alone does not establish the funding arrangement.
Before making a home decision
Ask care, legal and financial professionals how your circumstances affect timing and funding. Do not assume your home must be sold before exploring these questions. Keep a property plan flexible rather than treating an uncertain admission date as a confirmed moving date.
